Lilly disclosed 80-week Phase III TRANSCEND-T2D-2 topline results on 28 October: retatrutide improved A1C and weight more than semaglutide 2 mg in adults with inadequately controlled type 2 diabetes. The investigational triple agonist is not yet approved.<\/p>\n

In 1,298 randomized participants, 9 mg and 12 mg retatrutide lowered A1C by 2.2 percentage points versus 1.8 with semaglutide; weight fell 19.5% and 20.9% versus 9.4%. The primary objective was A1C noninferiority for 12 mg; selected superiority tests met multiplicity control. Adverse-event discontinuation was 4.8% and 7.9% versus 2.7%, and dysesthesia 4.8% and 6.5% versus 2.7%. The trial was open-label with 2:2:1 randomization.<\/p>\n

Deep Analysis<\/h4>\n

The active head-to-head comparator and long treatment period provide useful competitive evidence. But semaglutide 2 mg is not the full current obesity or diabetes comparator set, and the report uses an efficacy estimand that models continued treatment without rescue medication. Real-world persistence, cardiovascular outcomes, lean-mass effects and severe-event rates are needed to translate mean weight change into net benefit. Higher discontinuation at 12 mg may matter despite the larger weight effect. Lilly has not yet published full peer-reviewed results.<\/p>\n
