In heavily pretreated relapsed osteosarcoma in China, the B7-H3 ADC risvutatug rezetecan improved central-review PFS against gemcitabine–docetaxel. Safety burden and immature survival limit the net-benefit conclusion.

Among 122 randomized patients, median PFS was 5.5 versus 1.4 months (HR 0.33, 95% CI 0.20–0.55; P<0.0001) at 7.1 months median follow-up. Response rates were 15% versus 2%. OS was immature (HR 0.51, 95% CI 0.21–1.23). Grade-3-or-higher treatment-related events occurred in 93% versus 83%, and serious treatment-related events in 64% versus 29%; one grade-2 treatment-related ILD case resolved. Hansoh reports 2:1 randomization, consistent with 80 versus 42 reported participants; GSK’s release says 1:1, an unresolved source discrepancy. China accepted a BLA and gave priority review on 24 September; this is a data report, not a new approval.

Deep Analysis

Randomization, active chemotherapy control and blinded PFS review make the disease-control signal credible. But median OS is not reached and its interval includes no benefit. The highly myelosuppressive profile may matter particularly in young patients with prior intensive chemotherapy. Dose choice at 12 mg/kg was intended to address bone penetration, yet higher exposure raises toxicity questions. China-only enrollment, local care pathways and a selected late-line population constrain global extrapolation. GSK owns ex-China rights and is studying sarcoma internationally. The inconsistent allocation descriptions should be corrected by the sponsor and trial record before detailed subgroup weighting.

Company and Product Background

Risvutatug rezetecan (HS-20093/GSK5764227) couples an anti-B7-H3 antibody to the topoisomerase-I inhibitor rezetecan via a cleavable linker. Relapsed osteosarcoma after multiple systemic regimens has few established options.

Signal Extraction

Interpretation A: B7-H3 delivery of a topoisomerase-I payload offers real activity in refractory osteosarcoma; controlled PFS supports it.

Interpretation B: early progression control carries substantial marrow toxicity without proven survival benefit; event rates and immature OS support this.

Upgrade with mature OS, patient-reported benefit, dose intensity and independent global replication. Downgrade with cumulative cytopenia or poor efficacy at a tolerable dose. Falsification: a modern global trial shows no meaningful PFS or survival advantage.

InSilens Take

Inference: ARTEMIS-011 strengthens the B7-H3 ADC thesis in China, while clinical displacement outside China remains unproven.

Signal Assessment

Signal Importance: 5 of 5. Signal Direction: mixed (PFS gain and substantial toxicity). Confidence in Facts: high except allocation discrepancy. Confidence in Interpretation: moderate.

Hansoh disclosed full Phase III ARTEMIS-011 results on 27 October, following the 26 October ESMO presentation.
