A US priority-review application is pending; this is not an approval.

The 776-patient randomized trial compared the TROP2 ADC with doxorubicin or paclitaxel. At 14.0 months median follow-up, median OS was 19.0 versus 12.1 months (HR 0.68, 95% CI 0.55–0.84; P<0.0001), and blinded-review median PFS was 7.2 versus 3.9 months (HR 0.59, 95% CI 0.50–0.70; P<0.0001). Response rates were 35.1% versus 17.3%. Grade-3-or-higher treatment-related adverse events were 54.4% versus 41.4%; anemia, neutropenia and stomatitis were prominent. Three treatment-related deaths occurred in each arm. The FDA accepted a BLA for priority review with a 15 June 2027 target action date.

Deep Analysis

Concordant survival and tumor-control endpoints in a global randomized population provide substantial patient-benefit evidence beyond a single radiographic endpoint. The 6.9-month median OS difference is more consequential than the 3.3-month PFS separation and merits analysis of downstream therapy, regional effects and censoring in the full paper. The control is a conventional post-immunotherapy chemotherapy choice, but treatment practice may evolve; no active ADC or targeted comparator was tested. The safety tradeoff is real: severe treatment-related events were 13 percentage points more common with the ADC, despite similar fatal-event counts. Assess dose intensity, transfusion and G-CSF use, treatment-related quality of life and cumulative oral mucositis. Kelun-Biotech developed sac-TMT in China and Merck leads the global program, making this a cross-border platform validation with direct global trial evidence. The Lancet online date is 26 October; issue metadata should be tracked separately.

Company and Product Background

Sacituzumab tirumotecan is a TROP2-binding antibody-drug conjugate with a topoisomerase-I inhibitor payload, developed with Kelun-Biotech. Advanced endometrial cancer can progress after platinum chemotherapy and immune checkpoint blockade, leaving limited established later-line options.

Signal Extraction

Interpretation A: targeted TROP2 payload delivery improves survival after platinum and immunotherapy; dual primary endpoints support it.

Interpretation B: the observed advantage partly reflects a weak later-line comparator and may diminish against future treatment sequences; no active novel-drug comparison supports caution.

Upgrade: full peer-reviewed subgroup and subsequent-therapy analyses, regulator review and real-world tolerability. Downgrade: severe cytopenia or mucositis burden offsets patient benefit in practice. Falsification: benefit is not reproduced in a contemporary comparator population.

InSilens Take

Inference: TroFuse-005 provides strong global evidence for sac-TMT in this defined later-line setting, while net tolerability and future comparator relevance remain important.

Signal Assessment

Signal Importance: 5 of 5. Signal Direction: positive on OS and PFS; safety mixed. Confidence in Facts: high. Confidence in Interpretation: moderate to high.

The global Phase III TroFuse-005 study presented and published online on 26 October found both overall-survival and progression-free-survival gains for sacituzumab tirumotecan versus physician-choice chemotherapy after platinum and PD-1/L1 therapy in advanced or recurrent…

[3
244,1216,123,1107,1217
post-3905","post","type-post","status-publish","format-standard","has-post-thumbnail","hentry","category-therapeutic-indication","tag-endometrial-cancer","tag-kelun-biotech","tag-merck","tag-phase-3","tag-sacituzumab-tirumotecan
["GET
