The data were presented and published online in The Lancet on 26 October; overall survival remains immature.

At a prespecified interim analysis, 216 patients received iza-bren and 206 platinum-based chemotherapy. Median BICR PFS was 11.1 versus 5.7 months (HR 0.40, 95% CI 0.31–0.51; P<0.0001) after 14.6 months median PFS follow-up. Confirmed response rates were 60.2% versus 34.5%, with response durations 10.4 versus 5.7 months. The company reports a favorable but immature OS trend. Treatment-related discontinuation was 1.9% on iza-bren; adjudicated ILD was 0.9%, including one grade-3 case. Grade-3-or-higher treatment-related events were mainly hematologic.

Deep Analysis

The head-to-head randomized result and blinded assessment support a real disease-control effect against the tested chemotherapy. The concordant response and duration data strengthen the inference. But the regimen sits in a rapidly changing post-EGFR landscape that includes MET-selected EGFR combinations and other ADCs; cross-trial hazard ratios are not a valid ranking. Topoisomerase-I payloads can create cumulative marrow and pulmonary toxicity, so the low discontinuation figure should be evaluated alongside dose intensity, cytopenia severity and longer ILD surveillance. The trial was conducted in China by Biokin, SystImmune’s parent; BMS jointly develops the asset outside China. Regional portability requires a global comparator and modern prior-treatment mix. The Lancet online date is 26 October; issue assignment should be tracked separately.

Company and Product Background

SystImmune and Biokin developed izalontamab brengitecan, also BL-B01D1, a bispecific EGFR/HER3 antibody-drug conjugate carrying a topoisomerase-I inhibitor. EGFR-mutant NSCLC often progresses after third-generation EGFR TKIs. BMS holds joint development rights outside China.

Signal Extraction

Interpretation A: dual EGFR/HER3 binding improves payload delivery after kinase resistance; randomized PFS supports this.

Interpretation B: benefit reflects a chemotherapy comparator that may be displaced by selected targeted regimens, with unproven OS; evolving practice and immature survival support caution.

Upgrade: mature OS, full peer-reviewed safety and global replication against contemporary care. Downgrade: cumulative ILD or marrow toxicity, or a weaker effect after modern first-line combinations. Falsification: a global randomized study fails to improve a patient-relevant endpoint.

InSilens Take

Inference: iza-bren has a credible post-TKI competitive position in the studied China population, but survival, sequencing and global benefit are unresolved.

Signal Assessment

Signal Importance: 5 of 5. Signal Direction: positive for PFS; OS uncertain. Confidence in Facts: high. Confidence in Interpretation: moderate.

The China Phase III PANKU-Lung01 trial reported a substantial blinded-review PFS gain for the EGFR×HER3 bispecific ADC izalontamab brengitecan versus platinum chemotherapy after EGFR TKI progression.
