The randomized 464-patient G-HOPE-001 trial of Innovent-originated, Takeda-licensed arcotatug tavatecan met both overall survival and progression-free survival endpoints against physician\u2019s-choice chemotherapy in CLDN18.2-positive advanced gastric or gastroesophageal junction cancer. The mature overall-survival result is the central signal; global treatment positioning remains untested.<\/p>\n

On 23 October 2026 Takeda disclosed the second prespecified interim analysis, cutoff 3 September 2026: median overall survival 10.2 versus 6.9 months (HR 0.57, 95% CI 0.45\u20130.73; P<0.0001), and independent-review median progression-free survival 7.1 versus 3.1 months (HR 0.45). Confirmed response was 37.4% versus 7.7%. Grade 3 or higher treatment-related adverse events were 47.4% versus 55.9%; one treatment-related fatal sudden death of unknown cause occurred on ADC.<\/p>\n

Deep Analysis<\/h4>\n

Randomization, an overall-survival endpoint, and substantial effect size make this stronger than typical single-arm ADC evidence. Enrollment was in China and Japan, after two to four prior regimens, with strict CLDN18.2 IHC positivity (2+ in at least 75% of tumor cells). The comparator was irinotecan, paclitaxel, or trifluridine\/tipiracil. Thus the result does not establish first-line use, benefit in lower expressers, or superiority to other CLDN18.2 agents. More anemia with the ADC and the treatment-related death warrant full exposure-adjusted safety review. A companion assay, geographic practice patterns, prior treatment mix, and subsequent therapy will matter for regulatory portability. Innovent has a China NDA under priority review; Takeda plans a Japan filing and further global Phase 3 studies. Neither is an approval.<\/p>\n
