Iovance\u2019s final 24-patient Phase 2 cohort of lifileucel plus pembrolizumab in untreated advanced melanoma shows 62.5% confirmed response, 29.2% complete response, and median overall survival not reached after 50.9 months median study follow-up. Durability is notable, but this is uncontrolled evidence.<\/p>\n

The October 23 ESMO presentation reported investigator-assessed responses in 15 of 24 treated patients meeting product specifications. Median progression-free survival was 19.8 months. Median response duration was not reached at 26.9 months median response follow-up. The ongoing randomized TILVANCE-301 compares the combination with pembrolizumab alone and has response and progression-free survival co-primary endpoints.<\/p>\n

Deep Analysis<\/h4>\n

Extended follow-up limits the concern that responses are merely transient and makes the regimen worthy of randomized testing. Yet the cohort is small, selected, and lacks a contemporaneous comparator. Cross-trial comparisons with checkpoint combinations are confounded by baseline risk, subsequent therapy, assessment schedules, and censoring. The package includes tumor procurement, lymphodepletion, cell expansion, infusion, and supportive care; these affect feasibility and patient selection as well as safety. \u201cMedian OS not reached\u201d depends on the Kaplan\u2013Meier curve and number at risk, not simply a four-year survival guarantee. The existing prior-line approval of lifileucel does not validate first-line combination benefit.<\/p>\n
