Favorable subgroup and adjusted estimates are hypothesis generating and cannot override the failed primary comparison.

Elicio presented 144-patient results and a peer reviewed Nature Medicine paper on 23 October 2026. Baseline circulating tumor DNA positivity was 20.8% in the vaccine arm versus 12.5% under observation. The company reported a multivariable adjusted HR of 0.47 and nominal subgroup HRs of 0.51 in ctDNA-negative patients (n=93) and 0.57 in ctDNA-positive patients (n=26). The ctDNA-positive control subgroup comprised six patients. Grade 3 or higher adverse events were 10.4% versus 14.6%.

Deep Analysis

The trial tests a biologically plausible off-the-shelf, lymph-node-targeted mutant-KRAS peptide plus CpG adjuvant strategy in a high-risk setting. Nearly universal measured KRAS-specific T cell responses support target engagement, but immune responses are surrogate measures and stronger responses may reflect patients with better underlying prognosis. Baseline ctDNA imbalance can explain part of the unadjusted contrast, yet adjusted analyses rely on modeling and subgroup p values are nominal. Observation is a clear comparator following standard local therapy, but a future registration strategy needs prespecified stratification, a reliable clinical assay, sufficient power, and a treatment schedule justified by pharmacodynamics. The trial does not demonstrate efficacy in the full intended population.

Company and Product Background

Camupepimut (ELI-002 7P) combines seven mutant-KRAS peptide antigens with an amphiphile-modified CpG adjuvant intended to traffic to lymph nodes and prime T cells. It is being studied after curative-intent treatment for KRAS-mutated pancreatic ductal adenocarcinoma, a disease with frequent relapse.

Signal Extraction

Interpretation A: chance prognostic imbalance obscured a real vaccine effect; consistent subgroup direction and adjusted estimate support continued testing.

Interpretation B: the vaccine induces immunity without sufficient clinical impact; failure of the primary endpoint and very small high-risk control group support this.

Upgrade: a prospectively stratified randomized trial meeting its primary endpoint. Downgrade: subgroup effects attenuate with longer follow-up or independent adjustment. Falsification: an adequately powered trial reproduces immune responses but no disease-free survival gain.

InSilens Take

Inference: further development can be justified as a testable thesis, but this readout lowers confidence in broad adjuvant efficacy. Any Phase 3 should be framed as a new validation attempt, not confirmation.

Signal Assessment

Signal Importance: 4 of 5. Signal Direction: mixed. Confidence in Facts: high. Confidence in Interpretation: low to moderate.

Randomized Phase 2 AMPLIFY-7P did not meet its prespecified intent-to-treat disease-free survival endpoint for camupepimut after pancreatic cancer treatment (HR 0.85).
