Detailed randomized Phase 3 results strengthen the case that individualized neoantigen vaccination can add to adjuvant PD-1 blockade in resected high-risk melanoma. The observed benefit is in recurrence and distant-metastasis endpoints; overall survival and real-world delivery remain unresolved.<\/p>\n

On 22 October, Merck and Moderna disclosed the prespecified INTerpath-001 analysis in 1,137 randomized patients with resected stage IIB\u2013IV melanoma. At 24.6 months median follow-up, recurrence-free survival favored intismeran autogene plus pembrolizumab versus pembrolizumab alone (HR 0.58, 95% CI 0.45\u20130.75; p<0.001); distant-metastasis-free survival HR was 0.62 (0.45\u20130.85; p=0.002). Two-year recurrence-free estimates were 82.2% versus 70.0%. Results were announced for ESMO presentation and accepted for journal publication; this release precedes the full paper.<\/p>\n

Deep Analysis<\/h4>\n

Randomization and a clinically consequential endpoint make the efficacy signal stronger than prior single-arm neoantigen vaccine evidence. The first six months showed little separation (RFS HR 0.99); a prespecified post-immunization sensitivity analysis showed HR 0.42, consistent with delayed biological action but also vulnerable to time-dependent interpretation. Grade 3\u20135 treatment-related adverse events were 14.2% versus 13.4%, although longer follow-up is needed. The individualized manufacturing chain includes tumor sequencing, candidate selection, synthesis, release testing and timely delivery; those steps could limit applicability even if trial efficacy holds.<\/p>\n
