The decision extends a clinically validated PD-1 plus ADC regimen into an EU perioperative population that includes cisplatin-eligible patients.

The 21 October decision followed KEYNOTE-B15/EV-304, a randomized phase 3 comparison with neoadjuvant gemcitabine-cisplatin and surgery in cisplatin-eligible disease. Event-free survival favored the combination (HR 0.53, 95% CI 0.41–0.70); overall survival also favored it (HR 0.65, 95% CI 0.48–0.89). The EU had previously authorized the regimen for cisplatin-ineligible resectable disease on a separate trial. This is a regulatory action, not new clinical data.

Deep Analysis

A survival advantage over active neoadjuvant chemotherapy changes the evidentiary basis for using antibody-drug conjugates in potentially curable bladder cancer. PD-1 blockade may amplify antitumor immunity while the Nectin-4-directed ADC delivers a microtubule-disrupting payload, but the trial establishes regimen benefit rather than mechanistic synergy.

The relevant alternative is that clinical uptake may be limited by neuropathy, rash, hyperglycemia, perioperative scheduling, patient fitness and cost even with a positive benefit-risk determination. Cross-trial comparisons cannot identify which patients need both agents or the optimal duration of postoperative treatment. Longer survival, surgical completion and real-world toxicity will clarify displacement of cisplatin-based pathways.

Company and Product Background

Pembrolizumab blocks PD-1 immune inhibition. Enfortumab vedotin is a Nectin-4 antibody-drug conjugate carrying monomethyl auristatin E. Muscle-invasive bladder cancer can be cured surgically in some patients, and perioperative systemic therapy aims to reduce recurrence and death.

Signal Extraction

Fact: European Commission authorization; randomized EFS and OS benefit underlies it.

Inference: the regimen may displace neoadjuvant cisplatin in eligible patients where feasible.

Downgrade: material treatment discontinuation or surgical delays in routine use, or narrower benefit in key subgroups.

InSilens Take

Inference: this is an EU access expansion with a high-quality randomized basis. Optimal patient selection and perioperative tolerability remain implementation questions.

Signal Assessment

Signal Importance: 4 of 5. Signal Direction: Positive. Confidence in Facts: High. Confidence in Interpretation: Moderate–High. The direction score describes the evidence at this stage. It does not imply clinical approval, treatment readiness or commercial outcome.

The European Commission authorized pembrolizumab plus enfortumab vedotin before cystectomy and continued afterward for adults with resectable muscle-invasive bladder cancer.
