A chemoresistant aggregate termed LR2-high identified approximately 20% of patients and was associated with five-year event-free survival below 40%; among 115 known non-transplanted LR2-high patients, reported five-year event-free survival was 16%. Combining resistant-state abundance with cytogenetic risk and measurable residual disease improved outcome prediction. Functional studies nominated vulnerabilities including FLT3, CDK4\/6, retinoid signaling and transcriptional regulators; tamibarotene response prediction correlated with viability across four samples.<\/p>\n<h4>Deep Analysis<\/h4>\n<p>Relapse in AML can arise from minority states that are diluted in bulk profiling. Measuring those states at diagnosis could complement cytogenetics and MRD, particularly when high-risk biology is not captured by dominant clones. Yet discovery in 33 paired cases creates overfitting risk, rare populations near five percent challenge assay reproducibility, and treatment\/transplant selection can confound survival associations. The drug-matching experiments are preclinical and small; correlation across four samples cannot define a predictive biomarker. Clinical translation requires a locked classifier, reproducible sampling, turnaround compatible with induction decisions and prospective utility beyond existing risk tools.<\/p>\n<p>Interpretation A: rare resistant states are causal reservoirs and actionable biomarkers. Enrichment at relapse and functional drug sensitivity support this; retrospective design and treatment confounding weaken causality. Interpretation B: the signatures mark broader adverse biology without adding clinically deployable information. Their association with outcome supports prognostic relevance, while incremental multivariable performance argues they are not merely redundant.
