Antiphospholipid syndrome is an autoimmune thrombo-inflammatory disorder; persistent lupus anticoagulant identifies a pregnancy subgroup at particularly high risk for fetal death, severe preeclampsia and placental insufficiency.<\/p>\n<p>Certolizumab pegol is a pegylated Fab fragment that neutralizes TNF-alpha and lacks an Fc region, minimizing active placental transfer. In APS, the proposed action is suppression of TNF-linked placental inflammation alongside anticoagulation. This mechanism differs from preventing thrombosis and may address a residual pathway, but systemic TNF blockade carries infection and malignancy warnings.<\/p>\n<h4>InSilens Take<\/h4>\n<p>This is a clinically important designation because it advances an anti-inflammatory strategy in a high-risk pregnancy population that still experiences severe outcomes despite heparin and aspirin.
