Negative and positive predictive values were 94% and 85%, respectively. The tracer directly binds amyloid and showed performance across light-chain and transthyretin disease in secondary analyses. Results were published online in JAMA and presented at ESC. Evuzamitide remains investigational; the single-group referral-cohort design, adjudicated rather than uniform biopsy reference, false-positive rate and iodine-124 logistics constrain immediate clinical claims.<\/p>\n<h4>What Happened<\/h4>\n<p>REVEAL enrolled 195 adults undergoing evaluation for suspected cardiac amyloidosis at 18 U.S. centers; 170 were included in the primary analysis, and patients with an established cardiac or systemic amyloidosis diagnosis were excluded. Three blinded PET readers assessed cardiac uptake, while three blinded amyloidosis experts independently adjudicated diagnosis using the standard-of-care algorithm. Scans occurred three to five hours after intravenous administration of 1 mCi iodine-124 evuzamitide, with oral potassium iodide for thyroid blockade. Reader-specific sensitivity ranged from 90% to 94% and specificity from 84% to 89%. Only one treatment-emergent adverse event, fatigue, was considered tracer-related, with no deaths or related discontinuations reported. Bayer acquired AT-01 from Attralus, holds FDA Breakthrough Therapy designation and U.S.\/EU orphan status for AL and ATTR imaging, and plans regulatory discussions.<\/p>\n<h4>Deep Analysis<\/h4>\n<p>Current radionuclide algorithms are strongest for transthyretin cardiac amyloidosis and require monoclonal-protein testing to avoid misclassifying AL disease; direct fibril imaging with high negative predictive value could shorten diagnosis and support biopsy targeting or systemic-burden assessment across subtypes. The counterargument is that uptake does not type the amyloid precursor, so laboratory and tissue methods would still be needed, and 86% specificity implies clinically meaningful false positives. Blinded central reads and a prospective multicenter design support robustness, yet a selected referral population, expert adjudication and exclusion of previously diagnosed cases may not reproduce community prevalence or spectrum, and iodine-124 availability, long half-life, radiation handling, scan timing and thyroid blockade all affect workflow and economics. Diagnostic value therefore depends on external validation and whether the test changes time to treatment, biopsy use or outcomes.<\/p>\n<h4>Company and Product Background<\/h4>\n<p>Bayer acquired iodine-124 evuzamitide, also known as AT-01, from Attralus in 2026 as part of a molecular-imaging expansion. The peptide radiotracer is designed to bind the beta-sheet architecture shared by diverse amyloid deposits, allowing PET visualization of cardiac and potentially extracardiac burden. Cardiac amyloidosis results from extracellular fibril deposition that thickens and stiffens the myocardium; ATTR derives from transthyretin, while AL disease arises from immunoglobulin light chains and can progress rapidly.
